This is my favorite idea book as it is full of photographs of many school projects in Great Britain. It takes a wider view than most American resources, looking at the many possibilities for education outdoors in all subjects, and focusing on the design of human-friendly as well as animal friendly landscapes.

A shorter, less expensive resource similar to the Outdoor Classroom listed above. Includes a step-by-step planning guide, very thoughtfully done.

Habitats for Learning, A Planning Guide for Using and Developing School Land Labs. 1995.
Ohio Department of Natural Resource. 614-265-6878.
The Ohio approach is different then any other I've seen, calling for schools to first Use, then Enhance, and lastly Develop our outdoor sites. The arguments are pretty compelling. A companion video comes with this. Ohio's list of "land labs" is impressive, with a full spectrum of projects from prairies to wetlands.

Homes for Wildlife A Planning Guide for Habitat Enhancement on School Grounds.

Specifically about creating animal habitat, this binder is comprehensive, specific, and cheap. It's especially strong in that front-end aspect of wildlife projects, the mapping and inventorying of your site.

Schoolyard Habitat Planning Guide.
National Wildlife Federation.
This pamphlet is a good resource if you just want a brief but well-designed overview with some suggestions on where to look further. Focuses on wildlife habitat creation.
